Our 3^rd Annual Benefit outing will be held Saturday July 14^th 2012 
At Sycamore Country Club

Format:        Four man scramble 

Time:           10 am Shotgun start   

Cost:            $100. Per player includes Golf, Beverages, Food at the turn, and Dinner

Registration: Advance registration is recommended as space will be limited. Registration forms and checks should be in no later than July 1^st, 2012^.. Please
mail all registration forms and checks to IUEC local 35, 890 Third Street
Albany, NY 12206.                                                             

Checks should be made payable to “IUEC Local 35 Golf outing”

Foursomes: We would prefer that you sign up as foursomes but if you wish to sign up as Individuals or doubles we will do our best assign you to a foursome.

Hole Sponsors and other donations will be greatly appreciated for
contributing to the success of our tournament. If interested please return the completed registration/sponsorship form to IUEC local 35.
